CLE Forms

COURSE SPONSORS can now apply for courses online. Click on sponsor login on left hand side of page.  Contact us for your sponsor ID and password at oasis@mbcle.state.mn.us.

LAWYERS need to complete and submit a completed course accreditation form if they are submitting a course for review.

Appendix I: CLE Course Accreditation Application (Course Approval Form) -

To apply for accreditation (course approval), download and complete the CLE Course Accreditation Application and send to the MBCLE office at the address on the form.  Please provide the information called for by the form including detailed schedules showing the start and stop times spent in each session, the subject matter addressed during each session, and faculty , as well as a check for $35 with each course accreditation application submitted. Courses 60 minutes or less are exempt from this fee.

Appendix II: Affidavit of Pro Bono Representation -

After completion, the Affidavit of Pro Bono form may be submitted electronically by scanning the completed form and emailing it to attendance@mbcle.state.mn.us.

Appendix III: Affidavit of CLE Compliance -

NOTE: Effective 7/1/2010 there will be a $10 fee for filing paper affidavits of attendance.  There is no charge if you report your CLE attendance online through OASIS.
